The program provides community inclusion for individuals with disabilities to fully participate in the community and enhance their independent living skills. Choices staff support the individual’s choices in learning or utilizing their social skills while participating in community-based activities. Planned activities may include shopping, socialization, volunteering, or daily life skills.
Personal Assistance
Personal assistance may include bathing, toileting, transfer and ambulation, skin care, grooming, hygiene, use of adaptive equipment, household chores, or any person-centered plan designed to help individuals with their daily life skills. All Community Services are funded and available through Department of Mental Health or Phelps County SB-40 Services.
