Based upon the individual needs of the person with disabilities, the program offers a variety of services and supports to ensure the health and safety of the individuals who live in their own homes in the community. Choices for People offers up to 24-hour staff supported care and services. Services and supports include rehabilitation, nursing, behavioral support, daily living skills, home care, personal care, and community inclusion. Community involvement and social interaction empowers individuals to live fulfilling independent lives. This program is funded through the Department of Mental Health.
Level of care may include instruction in, or assistance with daily living skills, home care, personal care and community inclusion. Community involvement and social interaction are emphasized for each member of the home.
